Resident Youth Participation Application

All participants must register each year for a Resident Youth Card. A completed Participant Information Form and proof of residency are required for registration. The first card is free and each replacement card costs $5.00.

The application, risk agreement, behavior contract and computer use contract are available for download and printing to complete.  This application is required prior to using the Teen Center.

TEEN CENTER TRANSPORTATION SERVICE TEEN CENTER TRANSPORTATION SERVICE

The Casa Grande Parks and Recreation Department is offering transportation to the Teen Center from Casa Grande Middle School, Cactus Middle School and Villago Middle School. The transportation will take place immediately following school at 4 PM. The students will be dropped off at the Parks and Recreation Office (404 E Florence Blvd) at 7 PM for pickup. Each schools day for transportation is as followed: Casa Grande Middle School – Monday, Cactus Middle School – Tuesday, Villago Middle School – Thursday. If your student wishes to participate in this program they can pick up a Teen Center application at the front office of their school. After the application is filled out by a guardian they must return it to the front office. For any additional questions contact the Parks and Recreation Office at 421-8677 ext. 4560.
