How to File a Discrimination Complaint
An individual may file a written discrimination complaint with the Casa Grande City Attorney’s Office, 510 E. Florence Blvd., Casa Grande, AZ 85122 by using the Casa Grande City Attorney’s Office “Civil Rights Complaint Form (PDF).” The complaint shall be accepted and reviewed by the Human Resources Department Director. In the event that the Human Resources Director does not have the capacity to investigate the discrimination complaint, the complainant shall be notified. The complaint will be referred to the Office for Civil Rights or the Arizona Department of Public Safety or the complainant shall have the option to directly file a complaint with either the Office for Civil Rights or the Arizona Department of Public Safety at the listed addresses:

In order to file with the federal Office for Civil Rights (OCR), two forms must be downloaded from the internet and submitted in writing to OCR. The forms can be found at https://www.ojp.gov/program/civil-rights/filing-civil-rights-complaint.
