Ambulance

The Casa Grande Fire Department (CGFD) proudly provides transport services through our paramedic ground ambulance program. Our paramedics are trained to the highest level in Arizona and strive to provide excellent patient care and the most rapid response times possible. In addition to four engine companies and a ladder/ladder tender company, our two paramedic ambulances allow the Casa Grande Fire Department to offer innovative, flexible, and quality services to our residents and visitors of the City the Casa Grande Fire Department will be adding additional ground ambulances in the very near future. 

These paramedic ambulances will currently be assigned to Fire Station 501 and Fire Station 504. Ambulances will respond with one of our Engine or ladder companies stationed throughout the City. The Department's ground ambulance program expedites the rival of the transport ambulance and aids in the rapid treatment and transport to the local emergency room. 

Emergency medical care, paramedic services, and efficient transportation for our residents have always been a priority for the Casa Grande Fire Department. By offering ambulance and transportation services, the EMS division of the Fire Department plans to actively enhance services to residents of Casa Grande. The Casa Grande Fire Department ambulance program was successfully implemented in November 2023 and continues to develop and grow. The goal of the Fire Chief and the EMS Division is to continually anticipate, evaluate, and meet the changing needs of our growing community and always provide the best possible service. 

The Casa Grande Fire Department currently provides emergency medical services for the City of Casa Grande at basic life support (BLS) and advanced life support level (ALS.) Adding these paramedic ground transportation units will strengthen our desire for a more timely response and transport. The City of Casa Grande will still utilize private ambulances provided by American Medical Response (AMR) as needed. AMR still has a certificate in the necessity (CON) for all parts of the City and the ability to function fully in the City of Casa Grande. 

Paramedics in the City of Casa Grande are trained to the highest level in Arizona. They can administrate all approved advanced skills in the Arizona Department of Health (AZDHS) paramedic expanded scope of practice.

Ambulance Billing

The EMS division is responsible for billing paramedic services provided by the Casa Grande fire department. Any person who needs paramedic attention and or transportation to a local emergency facility will be billed for the transport fee that is set by the Arizona Department of Health Services (AZDHS); the patient or the legal guardian (if the patient is under 18) is ultimately responsible for all bill charges. The City's Cost

Casa Grande and most other cities and municipalities fund emergency medical services, specifically Fire, EMS, and treatment, through a combination of taxes and general city revenues. The City of Casa Grande currently bills all service users an ambulance transportation fee (only for hospital transport.) This is not a change in practice other than the fact that previously, you would have been billed by the private ambulance company that provided the service. The only difference is that the City of Casa Grande now provides the transport service, so the bill will come from us.

Insurance Billing

Emergency medical responses are typically covered by insurance policies minus any deductible or copay. There are some restrictions, such as most always require transport to the nearest appropriate facility equipped to provide definitive medical care. 

The following is a breakdown in regards to various insurance providers:

Private Medical Insurance: Ambulance transport fees are typically covered. Follow the instructions on the bill and fill out the request for medical insurance information form (instructions provided below.) Upon receipt, your insurance provider will be billed. If any outstanding balance remains, you will be billed for the remainder. Your private provider determines all deductibles and copays; any questions related to those charges should be directed to your insurance provider.

AHCCCS: If the Casa Grande Fire Department receives the AZ AHCCCS information while at the hospital, that will be billed to AZ AHCCCS by our billing services.

Medicare/Plan B: If the Casa Grande Fire Department receives the Medicare information while they're at the hospital, that will be billed to Medicare by our billing services.

Non/Payment: The City of Casa Grande can offer a payment plan and a hardship waiver for those who cannot pay or do not have medical insurance (see below.) If you wish to authorize us to bill your insurance company directly, please fill out the required Request for Medical Insurance Information Form and mail it to:

If you wish, you can email the form directly to ambulancebilling@casagrandeaz.gov. The City will bill your insurance provider and provide you with a copy of the bill for any remaining balance, should one remain. You should direct any questions regarding your copays or coverage of fees to your insurance provider.

You may pay your bill by any of the following methods:

Telephone: Call (833) 469-7789 to pay by telephone. Credit card payments are accepted over the phone. We accept debit/credit cards with Visa and Master Card logos only. The credit card number and expiration date are required to process your credit card payment. Please have your E-Account number ready at the top left of the bill. The City's third-party vendor will charge a fee. 

Online Payment: This feature allows you to pay for ambulance services online. Please have your E-Account number ready at the top left of the bill. The City's third-party vendor will charge a fee.

Payment Options

The City of Casa Grande provides several payment assistance options. Customers may be asked to demonstrate a need to receive any of the accommodations listed below.

Payment Extension: Allows our customers extra time to pay a bill without incurring late fees. Please contact us at (833) 469-7789 to request a payment extension. 

Payment Plan: A customer can establish monthly payments to pay a bill over several months. Please contact us at (833) 469-7789 to establish a payment plan. 

Hardship Waiver: In very specific situations, fees related to ambulance billing can be waived, contingent upon supporting financial documents and approval of the City Attorney. Please complete the Declaration of Hardship Form and return it along with a copy of your last IRS W2 form (United States federal tax form issued by employers, which contains employee earnings) and your most recent bank statement by email to ambulancebilling@casagrandeaz.gov.
